Call Off Form (for members)
This form should be used for active members of the program that need to let us know that they will not be attending a specific day due to something unplanned and unavoidable.  In order to avoid a $5 No Call/No Show fee, this form must be completed PRIOR to your start time. Note that an email will be sent to staff with the date and time.

Please note that this should be a valid reason for missing and may require additional documentation in order to return. You should also remember that calling off will have an impact on your pay and may impact your ability to earn extra hours, leadership incentives, or attendance incentives.
